The patient, a 42-year-old homemaker from Jaipur, had wished to improve the shape of her nose for around two years, seeking a more balanced and refined facial appearance. A notable feature in this case was thick nasal skin. Thicker skin over the nose behaves differently in rhinoplasty, it can mask fine definition and tends to settle and refine more slowly during healing, which makes both the surgical technique and the recovery timeline important to plan for.

The patient presented with a clinically significant left-sided septal deviation alongside an external nasal deformity caused by years of contact trauma in MMA. His left nostril was substantially narrowed, and he was breathing predominantly through the right side. The external nose showed mild dorsal asymmetry and a slight deviation off the central facial axis. Both functional and aesthetic concerns were addressed in the same surgical plan, since correcting only the septum would have left the visible asymmetry untreated.

The patient presented with a mild dorsal hump and a wide, poorly defined nasal tip. The skin envelope over the tip was notably thick — a known clinical challenge in rhinoplasty because it limits how sharply the underlying cartilage framework can show through after surgery. The deformity was symmetrical, mild to moderate in severity, and primarily aesthetic in nature with no functional breathing complaint.
